Position: Program Assistant
Location: Mound, MN
Type: Permanent, full-time
Salary: $15/hr. – $16/hr. doe

Description: The program assistant will carry out and participate in programming, activities, and support in general to the clients at our adult day center. The program assistant will work directly under the Activities Director and work in collaboration with interns, volunteers, administration, management, and the owners to provide our clients with great days. The program assistant will encourage the mental, spiritual, emotional, and social growth of our clients.

Duties & Responsibilities:

  • Carry out daily programming responsibilities as assigned by Executive Director or Team Lead regarding small & large group activities including field trips.
  • Responsible for AM/PM snacks, meal setup, serving, clean up and assisting participants as identified including cutting food and feeding.
  • Responsible for ensuring that individual participants receive assistance (as identified in plan of care) with activities of daily living to include walking, bathroom assistance, meal assistance, escorting to physical, occupational or speech therapy etc.
  • Responsible for preparation, implementation, cleanup and evaluation of assigned daily programs, i.e., adult education, exercises, art therapy, horticulture therapy, intergenerational programs, etc.
  • Participate in daily A.M. staff meeting with Program Team.
  • Understand and uphold “Maltreatment of Vulnerable Adults Reporting Policy and Procedure”.
  • Maintain current certification in CPR/First Aid
  • Ongoing paid training on a quarterly basis
  • This position will require you to be on your feet, active, and engaged for most of the day
  • Perform other duties as assigned.

Minimum Qualifications:

  • Passion, skill and interest in working with the vulnerable adults.
  • Experience working in adult care required.
  • Current CPR and First Aid Certification required within 3 months of employment.
  • Class B or D Driver’s License required. Medication Assistance Certification preferred.

Position: Activities Director
Location: Mound, MN
Type: Permanent, full-time
Salary: $35,000 to $50,000 annually doe

Description: The Activities Director will bring their energy, creativity and imagination to this position! They will work directly with the owners to help create a vibrant and engaging environment for all our clients. The Activities Director is responsible for overseeing all client activities. This includes developing and coordinating successful activity programs, coordinating all internal and external resident activities, creating monthly activity calendars, arranging outside entertainment, create stimulating activities that promote mental, intellectual, creative, spiritual and physical well-being. The Activities Director will assess client needs, interests, plan, manage resources, run, and support activity program needs. The ideal candidate has leadership skills to lead a team of program assistants, operational savvy to think on their feet, will be able to design diverse activity curriculum, and can create programs to support, engage, and entertain aging adults and adults with physical, emotional and mental health issues. This position will require a vibrant and dynamic personality, with A LOT of energy, who will provide peace of mind through quality support to our clients and their loved ones and has a sincere passion for working with adults who have challenges with mental health, physical, and/or mental disabilities.

Duties & Responsibilities:
Activities Development

  • Develop, coordinate, implement, and lead a variety of programs and social groups
  • Facilitate cultural outings and sightseeing tours
  • Create flyers and sign-up sheets announcing upcoming events and activities utilizing MS Publisher, MS Word, and in-house television communication systems.
  • Accompany residents on trips when necessary.
  • Perform and direct the movement of residents to and from recreational areas.
  • Keep records of activities and resident’s activity attendance
  • Plan, develop and coordinate a comprehensive program of activities which meet the creative, physical, and social needs of the participants
  • Collaborate with multidisciplinary staff in developing activity programs
  • Develop organized means of collecting and storing activity concepts
  • Collaborate with staff and through surveys for input on activities
  • Tailor activities to incorporate the diversity of participants we serve
  • Collaborate with staff to ensure proper activity staffing
  • Research potential group projects in the community
  • Ensure appropriate activity placement for new participants
  • Give input into the activities budget for the center
  • Complete and submit monthly report outlining activity accomplishments
  • Schedule field trips


Care Plan Management

  • Follow established protocols for assigned care plans
  • Individualized, legible, and accurate progress notes written in conformance to statutes and contract requirements for assigned case load
  • Participate in care plan meetings for participants
  • Consistently work in cooperation with clients, employees, volunteers, and families to ensure Residents are receiving the best support


Program Management

  • Supervise program volunteers
  • Supervise staff when offsite for outings
  • Provide program management in Director’s absence
  • Conduct center tours in Director’s absence
  • Exercise judgment and discretion in analyzing and resolving problems
  • Maintain confidentiality; demonstrate sensitivity to and respect for a diverse population
  • Develop and maintain a positive, professional working relationship with staff, caregivers, and customers; serves as a role model of appropriate behaviors and interactions
  • Maintains compliance with state and federal regulations regarding medical record documentation and participation
  • Maintains knowledge and compliance with organizational policies, state and federal regulations, and accreditation standards pertaining to hospice care and other lines of business as applicable per community.
  • Develops, implements and monitors staffing plans based on organizational guidelines and in a fiscally responsible manner to meet patient needs and plan for census growth.
  • Manages licensing and abides by all laws, rules, and regulations


Minimum Qualifications:

  • Minimum requirements:
  • Four (4) year degree in social services, therapeutic or recreation therapy, psychology, nursing, or other related field of study
  • At least two years’ experience working with older adults, clients with dementia, mental illness, traumatic brain injuries, and other vulnerable adults.
  • 2-years’ experience of direct supervision of staff .
  • Demonstrate organizational leadership and professional interpersonal communication skills both written and verbal.
  • Ability to effectively communicate in English, both verbally and in writing.
  • Must have proficient computer skills.
  • Project, program, and budget development experience
  • Experience with managing a group setting

Position: Caregiver / Sojourn Assistant

We are currently seeking a crème de la crème Caregiver / Sojourn Assistant. A highly compassionate and personable individual open to working evenings, overnights and weekends on a consistent or as needed basis. Duties include providing exceptional care for our clients living in the Sojourn Suites, as well as those involved with Sojourn at Home. Individual may be asked to assist at the Sojourn Day Center to support with daily activities.
